About CHQI

The Centre for Healthcare Quality Improvement (CHQI), formerly known as the Ontario Health Performance Initiative of the Ministry of Health and Long-Term Care, was established as a division of The Change Foundation effective July 1, 2008. The Centre’s mission is to accelerate quality improvement in Ontario's healthcare system to improve outcomes in areas of provincial strategic priority. This will be achieved by initiating and leading quality improvement projects focused on provincial strategic priorities, and supporting providers and Local Health Integration Networks (LHINs) to improve specified indicators in these areas. Within the context of these projects CHQI will:
  • Work with senior leaders to incorporate quality as a core business strategy;
  • Develop the capability of local providers and LHINs to improve care processes and outcomes; and
  • Identify policy barriers to quality improvement and communicates these to appropriate individuals within the Ministry
The Board of Directors of The Change Foundation provides corporate governance for CHQI.
