Diabetic foot ulcer and venous leg ulcer care: focus for early implementation
Four Community Care Access Centres (CCACs) and their selected primary service providers are the first sites to adopt the Integrated Client Care model for services:
• Champlain CCAC and Carefor Health & Community Services/Ottawa branch
• Erie St Clair CCAC and Saint Elizabeth Health Care/Windsor branch
• Central West CCAC and Saint Elizabeth Health Care/Brampton branch
• North East CCAC and Bayshore Home Health/Sudbury branch
They will start by better integrating their wound care services — specifically, care related to diabetic foot ulcers and venous leg ulcers for clients with diabetes.
Phase 1: Testing and Implementation Stage
Supported by expertise from CHQI, the quality improvement work will take place over a
timeline (PDF) spanning approximately 18 months. After site visits in March and April to help each site prepare, CHQI will facilitate value stream mapping exercises, in which Improvement Teams analyze their work processes to find areas where they can better integrate their services and avoid waste and gaps in care. The teams will then design, test, and implement changes around the six key elements of the Integrated Client Care service delivery model:
• specialized case management
• coordinated multidisciplinary assessment
• systems navigation
• bundled reimbursement
• integrated clinical service delivery
• clinical best practice.
Through regular opportunities for action-based learning, sites will build capacity and capability for future quality improvement work.
Phase 2: Outcome and Impact Stage
For approximately 18 months after initial implementation, the client and health system outcomes will be formally evaluated. The project aims to create measurable improvement in five areas:
• clients’ health status and quality of life
• efficient use of healthcare resources
• the client/caregiver experience
• the CCAC/service provider experience
• alignment of incentives and reward of innovation.
During the same period, Local Oversight Groups will continue to monitor key performance indicators to ensure that they are holding the gains, while guiding the spread of changes to other branches in each CCAC.
